— BSA has new Medical Forms —
All scouts and adults who participate in Troop 26 activities need to download and complete the BSA medical form below at least once a year.
The 2019 printing of the BSA Medical Form is the latest version.
This form includes a portion that requires a doctor’s visit and exam with a portion to be completed and signed by a doctor.
If you are attending Philmont, be sure to use that form instead of the general BSA medical form. Keeping an extra copy for your personal records is also recommended.
